Educational Materials
Enhance your education activities with materials that can be used by the therapy team to prepare the patient and family/caregiver for discharge.

Educational materials include information regarding the disease, diet, medication management, home safety, adaptive devices, how to manage patient physical and mental changes (e.g. incontinence, mood swings, memory loss), travel, and much, much more.

The primary goal of Moving Forward is to prepare the patient for returning home.
