The Centers for Medicare & Medicaid Services and its contractors have a far-reaching ability to perform pre-payment and post-payment medical review of skilled nursing facility claims.  Part of the medical review and appeal process is the inclusion of an appeal letter that describes why the services provided were skilled and medically necessary.  Learn how to write an appeal letter that helps justify the care delivered and addresses the reason(s) for denial while avoiding common errors.
